Service Model Planning Assessment
Service Model Planning Assessments include a comprehensive assessment of the current model of service being provided to an individual. Drawing on the information provided by the assessment process (which commonly includes review of available file information, observation of and discussion with the client, interview with key stakeholders etc); analysis is conducted that considers the degree of alignment and implementation between current service elements and identified client need. A report is developed outlining the recommended model of service for the individual, with specific recommendations in key domain areas such as skills development, behaviour support etc, in order to maximise quality of life outcomes.
